Insurance
We have relationships with many dental insurers. We kindly ask that you provide your insurance information at the time you call to schedule your appointment. This is to allow enough time to check your coverage before your appointment.
Please call our office if your insurance provider is not listed below. Insurance providers often go by various names and affiliations, and we may be able to find coverage available to you that may not be listed.
We participate with the following dental insurance plans:
Aetna, Altus Dental, American General, Ameritas, Anthem Blue Cross, Assurant, Benemax, Blue Cross Blue Shield, Care First, Cigna, Connection Dental, Delta Dental, GEHA, Guardian, Humana, MassHealth (under 21 years of age), Metlife, Premera Blue Cross, Principal, The Children’s Medical Security Plan, Unicare, United Concordia Dental, and United Healthcare.
We are happy to check your insurance coverage as a courtesy to our patients.
However, we recommend that you contact your insurance provider directly to confirm your benefits and coverage before your appointment to avoid any confusion. Please review our Insurance Coverage Policy below for additional information about accessing and processing your insurance benefits.
Insurance Coverage Policy:
As a courtesy to our patients, our office makes every effort to review and verify insurance information before each appointment. However, some insurance providers may limit the information they make accessible, and it may not be possible to retrieve complete details of benefits and coverage.
- For the most accurate and comprehensive understanding of benefits, it is recommended that patients contact their insurance provider directly prior to their visit to confirm coverage and ask questions.
- Policies often include exclusions, deductibles, and copayments, and it is each patient’s responsibility to be informed about these individual terms prior to receiving services.
- After an insurance claim has been processed and paid by the insurance provider, there may be a remaining balance that the insurance provider has not covered. Any remaining or returned balance is solely the responsibility of the patient. Claims cannot be adjusted, backdated, or resubmitted after the claim has been paid.
- All copayments are due at the time of service.
